PATIENT BILLING & FEES

Horizon Anesthesia LLC offers you and your patients a simple payment structure. The following fee schedule and billing process have been developed to require minimal involvement from your administrative staff.

A minimum deposit of $595.00 is obtained prior to the date of service. This deposit covers the cost of drugs, equipment and the Anesthesiologist’s professional charge for the first hour of anesthesia care. The patient is subsequently charged $110.00 for each 15 minutes or portion thereof for any additional anesthesia time ($440.00/hour). This additional payment is either paid in cash or charged to the patient’s credit card after the anesthetic is completed and the total anesthesia care time is calculated. Anesthetic time is defined from the point in time when the anesthetic is administered to the time of transfer of care in the recovery area.Generally anesthesia time will be longer than the procedure time.

Example:

1 Hour 30 Minute Anesthetic = $595.00 (initial deposit prior to day of surgery) + $220.00 (30 minutes, charged on the day of the anesthetic) = $815.00 Total Charge

1 Hour 50 minute Anesthetic = $595.00 (initial deposit prior to day of surgery) + $440.00 (45+ minutes, charged on the day of the anesthetic) = $1035.00 Total Charge

Horizon Anesthesia LLC does not accept insurance or bill insurance carriers directly, nor do we accept Medicaid or Medicare. We are happy to assist patients in obtaining the maximum reimbursement allowed by their insurance carrier, if applicable. Horizon Anesthesia will mail an insurance reimbursement form to the patient or the patient’s family within 30 days of the anesthetic. This form will describe the exact nature of the anesthetic we performed, the reasons for the services we provided, and the appropriate CPT code (billing code). This form is then submitted by the patient or patient’s family to the insurance carrier.

At Horizon Anesthesia, our goal is to provide you with quality anesthesia services while minimizing the involvement of your office resources. Beyond supplying billing information for deposits and final payments, please direct all patient payment and billing inquires to us.
